Evidence of Educational Qualification: B.Pharm/D.Pharm Degree/Diploma certificate and all mark sheets. Necessary Pharmacy Council Registration Certificate. Evidence of Tamil Eligibility: 10th or 12th standard marks sheet. Proof of Residency (any one): Nativity Certificate issued by the Revenue Department Voter ID Panchayat / Municipality / Corporation Tax receipt Aadhar Card Ration Card Certificate of Character and Conduct: Issued by a Group A or Group B government officer (must be recent, issued within 3 months). Certified Evidence for Work Experience (Minimum 2 years required). No Objection Certificate (NOC) from the current employer, if applicable. For differently-abled persons, a fitness certificate from a Block Medical Officer.
