| Qualification | (1) Should have completed Degree in Pharmacy or Diploma in Pharmacy course from an institution having registration with Gujarat Pharmacy Council. (2) Candidate should be registered with Pharmacy Council of Gujarat. (3) Preference will be given to those having experience in drug dispensing work in hospital or dispensary. (4) Should not be more than 58 years of age. (5) Should have completed CCC level computer course from a recognized institution. |

Documents to be submitted
1. Age Certificate (Leaving Certificate / Adharcard / Pan Card / Birth Certificate / Election Card … etc).
2. H.Sc. Marksheet, if trial, all marksheets to be attached (HSC Marksheet).
3. Zeth. Disguise. C.Trial C (HSC Attempt Certificate).
4. Last Year/Last Two Semesters Marksheet of Pharmacy (If Semester then Marksheet of both Semesters must be uploaded as one PDF. If trial all marksheets must be copied) (Any Graduate Last Year/Last Two Sem. Marksheet).
5. Degree Certificate of Pharmacy.
6. Pharmacy registration certificate.
7. CCC Computer CD (Diploma / Certificate in Computer).
8. Experience Certificate (One PDF file of all experience certificates should be created and uploaded.
Terms and Conditions:
1. These posts are only on contract for 11 months. After 11 months the contractual posts will automatically end. Can’t claim for permanent job.
2. Appointed candidates cannot be transferred to other districts.
3. Only applications received by the candidate from the online link https://arogyasathi.gujarat.gov.in will be accepted. Applications received from RPAD/Speed ​​Post/Post/Courier or with incomplete details will be considered invalid.
3. All the necessary documents have to be uploaded as per the qualification mentioned in the advertisement. Applications with incomplete information or defects will not be considered.
4. A photocopy of the legible original certificate must be uploaded in the software.

7. Final decision on appointment will be subject to the prevailing conditions of Urban Health Society and National Health Mission.
