Special instructions regarding recruitment process –
1.1. The appointment and selection procedures for all the above posts are as per the SLP of the Honorable Supreme Court, New Delhi. (c) Will be subject to the final order number 19668/2022. In this regard, para 4, 5 and 6 of the interim order dated 01.05.2023 of the Honorable Supreme Court is as follows –
a. We therefore, permit the state to go ahead with the selection pocess and make appointment and promotions.
b. However, it is made clear that the same shall be subject to the result of the petition.
c. All appointment and the promotion order shall specifically mentions that such appointments and promotions are subject to the final outcome of the present proceeding.

Regarding application form:-
2.1. It is mandatory to fulfill all the educational and other qualifications on the date of advertisement, otherwise the application form will be rejected immediately. No correspondence will be accepted in this regard. Under no circumstances will any certificate be accepted after the last date for application. Therefore, make sure to attach all the certificates proving eligibility with the application, otherwise the application will not be considered.
2.2. Age Limit:- As on 01 January 2024, age should be 18 years and maximum 64 years for managerial post and 70 years for medical post. Age will be calculated as on 01 January 2024.
2.3. All posts are completely contract based, which is subject to the Contract Human Resource Policy 2018 (as amended) under the National Health Mission Chhattisgarh.
2.4. While submitting the application, the applicant must mention the designation, name of the program and name of the category (Unreserved/Other Backward Class/Scheduled Caste/Scheduled Tribe) in the application. Interested applicants should apply in prescribed format and self-attest all necessary documents and submit to Office Main
2.5. Medical and Health Officer, Ambikapur, Surguja by prescribed date/time. If the application is not received by the prescribed date and time after the issue of advertisement or any other.
2.6. The department will not be responsible for delay due to any reason or delay by post. No application will be accepted/considered in this regard nor will any representation be accepted.
2.7. Incomplete/unclear and erroneous applications will not be considered and unnecessary documents will not be attached.

2.8. The candidate should submit the application form and all the required documents together by tagging them (tying them in a thread).
2.9. Educational Qualification Certificate only. Government/UGC/AIISHI TE from an institution recognized by will be valid.
2.10. Chhattisgarh Valid, valid registration certificate from the Council (according to the relevant subject)/District Employment Office. It will be mandatory to attach.

Candidates will have to present with the application self-attested photocopies of the following documents and at the time of presence in the office after appointment, they will have to present with the original and one self-attested copy of the above mentioned records:
✓ Marksheet of 10 years (for proof of date of birth).
✓ 12th marksheet.
✓ Graduation/Post Graduation, Degree/Certificate (for all years).
✓ As per qualification, mandatory technical educational qualification related degree/diploma (for all years)
✓ Chhattisgarh Live registration certificate of the council (according to the relevant subject).
✓ Caste certificate given by the competent authority (if relevant).
✓ Caste certificate under Special Backward Tribe Category (PVTG) (if applicable).
✓ Domicile Certificate (issued by the competent authority).
✓ Live registration certificate of District Employment Office.
✓ If disabled, then Disabled Certificate (UDID) (issued by the competent authority)
✓ Self Identity Card (ID-Proof, Passport Size Photo) and Address Proof
✓ Other special documents (as required).
✓ Experience/No Objection Certificate: This will be valid only for government institutions, public institutions and government financially aided institutions (such as SHRC) or undertakings.
