Regarding application form/advertisement:-
1.1 This appointment is made by SLP of Honorable Supreme Court, New Delhi. (c) Will be subject to the final order number 19668/2022.
1.2 The format of the application form for the advertised post and the general conditions and guidelines regarding the recruitment of the said posts and the list of selected candidates and other information related to recruitment can be seen from the district website www.janjgir-champa.gov.in from time to time. And can be downloaded, and information will not be given through any other medium.
1.3 Application form along with self-attested mandatory documents should be sent by registered post/speed post to the office of Chief Medical and Health Officer, District Janjgir-Champa (Chhattisgarh) till 5.00 pm in office hours till 28.03.2024. Apart from this, application will not be accepted through any other means.
1.4 Application forms received after the prescribed date and office hours will not be accepted.
1.5 The candidate must be from Chhattisgarh. It is mandatory for the candidate to be a native of India, it will be mandatory for the candidate to attach the residence certificate issued by the competent authority.
1.6 Incomplete and erroneous applications will not be accepted. Instructions given in the outer advertisement to each applicant And after seeing all the information in the application form, fill the correct and complete information carefully. If in the application form. If any information is given incomplete or incomplete, the applicant will be completely responsible for the error or incompleteness. On the basis of this, the application form can be canceled at any stage of selection without prior notice to the applicant. The department will not be held responsible for postal delay if the application is not received by the prescribed date after issuing the advertisement.
1.7 At the time of applying for the above posts, it is mandatory to have the prescribed educational qualification (mark list of all semesters/all years) from a recognized education board/institution/university and computer related degree/diploma from a recognized university/institution.
1.8. Experience certificate issued by the employer will have to be submitted along with the application. In case of being currently employed, no objection certificate from the employer will have to be submitted and it is necessary to clearly mention the serial number and date of the issuing office in the certificate.
1.9 The name of the post applied for, category (Unreserved, SC, ST, OBC) should be clearly written on the envelope of the application submitted for the advertised vacant post. The name of the post is not mentioned on the envelope. In case of any finding, the selection committee will have the right to accept/reject the application.
1.10 It is mandatory to attach self-attested photocopy of the complete mark sheet of each year of compulsory/desired educational qualification along with the application.
1.11 In case of applying for more than one post, separate application and fee will have to be submitted for each post.

Document Verification:-
1. All the entries given by the candidates in the application should be correct as per the original documents. Final selection before or after document verification, it is found that the information mentioned in the application is incorrect or incomplete.
2. If at any time before/after selection, information from any party is found to be incorrect selection will be cancelled. 10.2 UGC, AICTE of the institution from which the educational qualification is obtained. And it is mandatory to get recognition from the concerned council.
3. At the time of written examination/skill test/interview and after selection, it is mandatory for the candidates to appear/present along with the original and one self-attested copy of the following documents:-
a. Marksheet of 8th.
b. Marksheet of 10th.
c. Marksheet of 12th.
d. Mark list of all the years of minimum educational qualification prescribed for the concerned post. Mark list of all years of desirable educational qualification for the concerned post.
e. Graduation/Post Graduation degree/certificate obtained from a recognized university.
f. Marksheet of Diploma/Degree in Computer from recognized institution.
g. Degree related to essential technical educational qualification as per qualification.
h. Concerned council/survival registration certificate.
i. Caste/residence certificate issued by the competent authority. (If necessary, it will be mandatory to submit a copy attested by the high level screening committee)
j. Domicile certificate of Chhattisgarh state.
k. identity card. (Aadhar Card/Voter ID Card/PAN Card/Driving License etc. in which Name, address, photo of the candidates)
l. If disabled (disability certificate)
m. Copy of work experience certificate and order.
n. Certificate for special discount.
o. No Objection Certificate from the present employer (if applicable)
p. Integrated Certificate in Community Health certificate for the post of Community Health Officer. Other required documents (if applicable). (Candidates should attach self-attested required documents with the application form as per the above order.)
