This recruitment process is strictly OFFLINE. Candidates must adhere to the following steps:

1. Obtain Application Form: The application form is available in the official notification document. Candidates must fill it out as per the prescribed format.
2. Fill the Form: Complete all sections of the application form accurately. Ensure that the post name and reservation category are clearly mentioned.
3. Prepare Demand Draft: Obtain a **non-refundable Demand Draft** for the application fee (Rs. 150/- for Unreserved, Rs. 100/- for Reserved categories) from a **Nationalized Bank**. The Demand Draft should be drawn in favor of “**Integrated Health and Family Welfare Society Sangli, Sangli Miraj and Kupwad City Corporation**”. Write your full name, applied post, advertisement serial number, and mobile number on the backside of the Demand Draft.
4. **Gather Documents:** Prepare attested (self-attested) photocopies of all necessary documents, including:
   • 10th Marksheet and Certificate (for Date of Birth proof)
   • 12th Marksheet and Certificate
   • Degree Marksheets (all years) and Convocation Certificate
   • Relevant Council Registration Certificate (e.g., Pharmacy Council, Paramedical Council, Nursing Council)
   • Two recent passport-sized photographs
   • Small Family Certificate
   • Caste Certificate (for Reserved categories)
   • Experience Certificates (if applicable)
   • Government Service Experience Certificate (if applicable)
   • Age, Nationality, and Domicile Certificate
   • Proof of Age (School Leaving Certificate, Birth Certificate)
   • Gazette, Marriage Registration, or Notarized Affidavit for name change (if applicable)
5. **Submit Application:** Submit the completely filled application form along with the original Demand Draft and attested copies of all required documents **in person** at the following address:

6. **Deadline:** Applications must be submitted between **10:00 AM and 05:00 PM** from **September 12, 2025, to October 03, 2025** (excluding holidays). Applications received after the deadline will not be considered.
