Application Steps:

• The application process is entirely OFFLINE.
• Download the official ECHS Application Form using the link below.
• Fill out the application form neatly and accurately.
• Affix a recent passport-size photograph in the designated space.
• Compile one set of duly self-attested photocopies of all documents as per the checklist below.
• Submit the completed application form along with all enclosures to the office of the concerned ECHS Stn HQ / Polyclinic before the last date.
• Candidates must carry all original documents for verification at the time of the interview.

Checklist of Documents to be Attached (Self-Attested Photocopies):

1. Application Form as per format
2. Aadhaar Card & PAN Card
3. 10th & 12th Certificate
4. B.Pharm / D.Pharm Degree or Diploma Certificate
5. Mark Sheets of all professional years/semesters
6. Valid Pharmacy Council Registration Certificate
7. Work Experience Certificates (Minimum 3 years)
8. Medical Fitness Certificate
9. For Ex-Servicemen: PPO, Discharge Book, ESM I/Card
10. No Objection Certificate (NOC) from the current employer, if applicable.
